If your student has a medical condition, allergy, or other health concern that the bus driver should be aware of, please complete the Bus Medical Form.

Once completed, either:

• Email the form to Karen Barnes at kbarnes@indiana.ag, or
• Visit the school office to complete a paper copy.

Providing this information helps us ensure your student's safety while riding the bus.
